What's your warn/ban policy?

My system can really depend on the offense, spam can get lighter warnings while illegal content can be the opposite, and such.

I don't have a systematic this equals to that, system.
I just give them the rules what I expect and what I don't from them.
But I do explain that they may receive verbal warnings, temporal suspensions and even permanent banning depending how severe we think it is and their attitude towards it.
I generally just give advice and warnings unless they are blatantly trying to break the forum order, disrespect members and staff and the like. Then usually a ban hammer is discharged on said user.
However, we tend to discuss in private what we will do with X member, if she or he is causing problems.

3 warnings = 3 or 5 day ban, depending on severity.
Another 2 warnings after that ban and it's a week ban.
1 more warning after that ban and it's permanent.
For bad offences i use straight bans.

Minor offences I'll tell them to stop. Then it becomes warings and then a ban. Those minor offences are like 1 word posts. Small offences are regular spam. I'll give them 3 warnings then a ban. Then stuff like triple posting or exessive spamming will get them banne dfor a week or so. Creating multiple acocunts will get the old acocunt ip banned for ahwile and the new one gets deleted. If its something like spamming the entire forum, posting porn, or hacking the Forum, there IP banned, account banned, and email banned forever.

I give 5 warnings for minor offenses via PM so they don't have any excuse of not knowing. I ban members after 3 successive major offenses. I use a warning system and indicated by a warning bar so they know the status of their account too.

I don't have a policy really, I just drain a warning bar after they violate rules multiple times.
